Intermittent fasting has become one of the most popular approaches to weight loss, with supporters claiming that limiting when people eat can provide benefits far beyond shedding extra pounds. Proposed advantages include better metabolic health, reduced risk of disease and even protection against cognitive decline and cancer.
But growing evidence suggests that some of these claims may have been exaggerated.
Intermittent fasting can involve restricting food to certain hours each day or fasting on particular days of the week. Although some studies suggest it can be effective for weight loss, others have found little difference compared with conventional calorie-restricted diets. Researchers also warn that the evidence in humans is far less convincing than the results seen in laboratory animals.
According to nutrition researcher Krista Varady, intermittent fasting generally produces around 5% weight loss in humans—roughly comparable to other dietary strategies. Even this relatively modest reduction can improve some metabolic indicators, including cholesterol and blood pressure.
There is also evidence that intermittent fasting may ease symptoms associated with polycystic ovary syndrome. However, more restrictive approaches, such as fasting every other day, could have disadvantages, including loss of lean muscle mass.
The gap between animal and human research is particularly striking. Experiments in mice have suggested that fasting can trigger a metabolic shift from using glucose to burning fat. This process may activate autophagy, in which cells break down and recycle damaged components. In animal studies, these mechanisms have been associated with healthier aging and protection against certain diseases.
Human studies have been much less conclusive. Research into possible effects on cognitive function has produced mixed results, while studies examining cancer have also failed to consistently reproduce findings from animals. One approach designed to imitate the biological effects of prolonged fasting appeared promising in mice but did not produce the expected metabolic response in humans.
There are some encouraging findings. A 5:2 fasting regimen may help some women with metastatic breast cancer tolerate chemotherapy better, potentially improving quality of life and slowing disease progression. However, such results do not mean intermittent fasting is broadly protective against cancer.
One of the biggest problems in studying diets is surprisingly basic: researchers often cannot determine exactly what people eat.
In animal experiments, scientists control every meal and calorie. Human participants, by contrast, are usually asked to record their food intake in diaries or questionnaires. These records can be incomplete or inaccurate because people forget what they ate, underestimate portion sizes or intentionally omit foods.
Research published in 2025 suggested that people may underestimate their food consumption by roughly 400 to more than 800 calories a day. In some cases, reported intake was dramatically lower than the amount of energy measurements showed participants had actually consumed.
New technologies could eventually improve the reliability of nutrition research. Researchers are testing cameras worn on glasses or necklaces, smart forks that monitor eating behavior and sensors attached to teeth that can detect nutrients. Blood and urine tests may also provide more objective information about what the body is processing.
The goal is to combine several of these technologies with traditional food diaries to create a much more accurate picture of people's diets while they continue living normally.
Researchers say future trials should also take into account physical activity and sleep, which can influence health outcomes and are carefully controlled in many animal experiments.
For now, the evidence suggests that intermittent fasting can be a useful strategy for weight management and may offer some metabolic benefits. But its effects in humans appear considerably less dramatic than those observed in animals, and there is not enough evidence to conclude that it can extend lifespan or prevent major diseases.
The popularity of intermittent fasting may therefore have outpaced the science. More accurate ways of tracking what people actually eat could help determine which benefits are real—and which have been overstated.
