A new review published in npj Cardiovascular Health highlights how genetics, hormones, lifestyle, and environmental factors influence blood pressure differently in women and men throughout life.
Researchers noted that while blood pressure naturally changes with age in both sexes, women tend to experience a much steeper increase beginning in midlife, especially during and after menopause. This rise may contribute to the higher cardiovascular risks observed in older women.
During childhood and adolescence, boys and girls generally have similar blood pressure levels. However, after puberty, women usually maintain lower systolic blood pressure than men throughout their reproductive years. Scientists believe female hormones such as estrogen and progesterone help protect blood vessels and support healthier blood pressure regulation during this period.
As women approach menopause, declining hormone levels may reduce this cardiovascular protection, leading to a faster increase in blood pressure. Researchers also pointed to genetic differences between the sexes, including the possible role of X and Y chromosome-related mechanisms in blood pressure regulation.
The review further emphasized that lifestyle and environmental stressors — including obesity, diabetes, smoking, unhealthy diets, lack of physical activity, chronic stress, pollution, and alcohol consumption — can significantly affect blood pressure. According to the authors, women may be more sensitive to some of these factors than men.
Scientists also discussed how certain medications and hormone therapies may influence blood pressure differently in women and men. For example, oral estrogen therapy after menopause may increase blood pressure risk, while transdermal estrogen appears to have a lower impact.
The researchers concluded that future prevention and treatment strategies for hypertension may need to become more personalized, taking into account sex, age, hormonal status, and lifestyle factors. They added that more studies are needed to develop tailored approaches for reducing cardiovascular disease risk.
