Artificial Intelligence Assists Conception: A New Era in Treating Male Infertility

Scientists from Columbia University Medical Center have reported the first successful pregnancy achieved using an artificial intelligence–driven sperm recovery technology. The study, published in The Lancet, marks a breakthrough in the treatment of male infertility.

The new method, called STAR (Sperm Tracking and Recovery), is designed to help men suffering from azoospermia — a condition in which no sperm cells are present in the ejaculate.

According to researchers, the male factor contributes to nearly 40% of infertility cases in couples, and 10–15% of men are diagnosed with azoospermia. Until now, the only option for obtaining sperm in such cases involved invasive surgical procedures that extract tissue directly from the testicles — methods that are not always effective and can lead to inflammation, vascular complications, or temporary decreases in testosterone production.

The STAR technology combines robotics, microfluidic systems, and artificial intelligence algorithms. The system scans millions of microscopic images of a cell sample, identifying single sperm cells hidden among cellular debris, and then isolates them with nanometer precision. The recovered sperm can be used for in vitro fertilization (IVF) or cryopreservation.

In its first clinical application, STAR achieved what had not been possible for nearly two decades: a man with severe infertility — who had undergone two failed surgeries and several unsuccessful IVF cycles — became a biological father. Within two hours, the system analyzed more than 2.5 million images from a 3.5-milliliter sample, detecting two viable sperm cells — enough to achieve successful fertilization and pregnancy.

Researchers emphasize that, while this is the first and only reported case so far, the technology opens a new frontier in the treatment of male infertility. Large-scale studies are now underway to confirm the method’s effectiveness and safety in broader clinical practice.
