Cocoa has long been linked to a variety of potential health benefits—from supporting heart health and slowing aging to improving cognitive function. However, experts caution that the picture is more complex than it may seem.
A number of studies associate cocoa consumption with positive health effects, largely due to flavanols—powerful plant-based bioactive compounds and antioxidants—and theobromine, a natural bitter stimulant found in cocoa beans and chocolate, Euronews reports.
That said, a bar of milk chocolate is not the same as raw cocoa. The risks and benefits can differ significantly depending on the type and processing.
Cocoa is rich in flavanols, a class of compounds also found in many plants and vegetables. These substances are associated with antioxidant, anti-inflammatory, and even anticancer effects.
Meanwhile, theobromine can cross the blood–brain barrier and act as an adenosine receptor antagonist, influencing neurochemical activity and potentially improving cognitive function, according to a 2024 study. This may offer protection against age-related cognitive decline, including Alzheimer’s and Parkinson’s diseases.
Cocoa has also been linked to a lower risk of death from cardiovascular causes.
A 2025 meta-analysis published in the European Journal of Preventive Cardiology found that flavanol-rich foods can help lower blood pressure and improve cardiovascular health in people with hypertension or elevated risk of heart disease.
Researchers suggest that similar benefits might be achieved by consuming one to two servings of dark chocolate or two to three tablespoons of cocoa powder per day.
In the Cocoa Supplement and Multivitamin Outcomes Study (COSMOS)—one of the largest and longest randomized controlled trials examining chocolate and health—more than 21,000 participants were followed for an average of three years.
Some participants took a supplement containing 500 mg of cocoa flavanols twice daily, while others received a placebo.
While cognitive health outcomes and rates of type 2 diabetes were similar between groups, those taking the supplement had a 27% lower risk of dying from cardiovascular causes.
However, researchers emphasize that COSMOS was not originally designed to evaluate the health benefits of chocolate itself.
“We were essentially asking whether there is something in cocoa beans and their bioactive compounds that could benefit health,” explained study co-leader Howard Sesso.
Scientists from King’s College London have also linked theobromine to slower aging. In a study of more than 1,600 participants, higher blood levels of the compound were associated with a lower biological age compared to chronological age.
Still, experts urge moderation. “Chocolate—even dark chocolate—is a treat, not a health food,” said Dimitrios Koutoukidis, a nutrition specialist at the University of Oxford.
If people enjoy chocolate, they should consume it occasionally and in small amounts, he added.
Doctors also note that commercially produced chocolate typically contains much lower levels of beneficial compounds than the cocoa products used in scientific studies.
Among common varieties, darker chocolate generally contains more flavanols and less sugar and added fat. White chocolate, by contrast, contains cocoa butter but no cocoa solids, making it the least beneficial nutritionally due to its high fat and sugar content.
It remains unclear how much chocolate is needed to achieve meaningful health benefits—or whether added ingredients like sugar, milk, and cocoa butter may offset them.
