Daughter’s wedding nearly caused a heart attack: doctors diagnose “happy heart syndrome”

A 65-year-old woman from Qatar was hospitalized with symptoms resembling a heart attack, but doctors discovered an unexpected cause: broken heart syndrome triggered by a joyful event, reports Live Science.

The woman had experienced intermittent chest pain for several months, but previous cardiac examinations had found no abnormalities. After her daughter’s wedding, however, her chest pain and shortness of breath intensified over three days, particularly during physical activity.

An electrocardiogram showed changes in the heart’s electrical activity, while a blood test revealed elevated levels of troponin, a protein released into the bloodstream when heart muscle is damaged.

An echocardiogram showed a sharp decline in the left ventricle’s pumping ability. Its ejection fraction had fallen from 58% five months earlier to 35%, while the apex of the left ventricle showed the characteristic outward bulging associated with takotsubo cardiomyopathy.

Doctors initially suspected a heart attack and performed coronary angiography. However, the arteries showed no significant blockages. Cardiac MRI also found no evidence of a previous heart attack or permanent tissue damage.

The patient was ultimately diagnosed with takotsubo cardiomyopathy, commonly known as broken heart syndrome. The condition causes the heart muscle to temporarily weaken and change shape, producing symptoms that can be almost indistinguishable from those of a heart attack.

When happiness affects the heart

Takotsubo cardiomyopathy usually develops after intense emotional stress, such as the death of a loved one, a serious medical diagnosis or another traumatic event. Scientists believe that a sudden surge of stress hormones, including adrenaline, temporarily disrupts the normal function of the heart muscle.

In this case, however, the trigger was not grief or fear but the intense joy of the woman's daughter's wedding.

This rare phenomenon is sometimes referred to as “happy heart syndrome.” It is a form of takotsubo cardiomyopathy in which strong positive emotions activate physiological stress mechanisms similar to those triggered by negative emotional experiences.

The woman was prescribed standard treatment for heart failure, and her condition subsequently stabilized. During follow-up, her ejection fraction had recovered to 56%, nearly returning to its previous level.

Takotsubo cardiomyopathy accounts for approximately 1–3% of acute coronary syndrome cases. Positive emotions are responsible for only a small proportion of emotionally triggered cases: according to the International Takotsubo Registry, they were associated with about 4.1% of such episodes.

Despite its nickname, “happy heart syndrome” is a real medical condition. The case also highlights that intense positive emotions can, in rare circumstances, place enough stress on the body to trigger temporary heart dysfunction.
