Eating large amounts of chili peppers may be associated with a higher risk of esophageal cancer, according to a review of scientific studies. However, researchers emphasize that the findings do not prove a cause-and-effect relationship and that further research is needed.
The meta-analysis, published in Frontiers in Nutrition, combined data from 14 observational studies involving more than 11,000 participants, including over 5,000 patients with gastrointestinal cancers. Researchers found that people with the highest chili pepper intake were about 64% more likely to develop gastrointestinal cancers overall than those who consumed the least.
The strongest association was observed for esophageal cancer. Individuals with the highest chili pepper consumption had nearly a threefold higher risk of developing the disease. In contrast, no statistically significant increase in the risk of stomach or colorectal cancer was identified, although stomach cancer showed a tendency toward a higher risk among heavy chili pepper consumers.
Chili peppers contain capsaicin, the compound responsible for their pungency. Previous laboratory studies have suggested that capsaicin may reduce inflammation and even suppress the growth of certain cancer cells. However, other research indicates that under some conditions it may promote tumor growth or contribute to chronic tissue irritation, leaving its overall role in cancer development uncertain.
The relationship between chili pepper consumption and cancer risk also differed across regions. Studies conducted in Asia, Africa, and North America generally reported a higher risk among heavy consumers, while studies from Europe and South America found little or no increase in risk. Researchers believe these differences may reflect variations in dietary habits, cooking methods, pepper varieties, genetics, smoking, alcohol consumption, and other lifestyle factors.
Scientists suggest that the esophagus may be particularly vulnerable because repeated exposure to very spicy foods could irritate its lining. However, this mechanism remains hypothetical and has not been confirmed.
The authors stress that all studies included in the analysis were observational, meaning they can only identify associations rather than prove that chili peppers directly cause cancer. Other factors, including smoking, alcohol use, infections, and overall diet, may also influence the results.
More recent reviews have reached mixed conclusions, suggesting that spicy foods may have both health benefits and potential risks depending on the amount consumed, the population studied, and the health outcome being examined. Some evidence even links moderate spicy food consumption to a lower risk of cardiovascular disease and premature death.
Researchers say future long-term studies are needed to determine whether moderate chili pepper consumption is safe and whether there is a threshold above which cancer risk begins to increase.
